Requirements for first time applicant
Seventeen
(17) years old and below
1.
Birth Certificate - NSO
2. Marriage Contract - NSO (If legitimate child)
3. Affidavit of Illegitimacy (If parents are not married)
4. Original School ID or minor (8 to 17 years old)
5. Original passport of either parent
6. If Illegitimate, Notarized affivadit of support
and consent and original passport or valid identification
card of mother is required, if father is one who appear
to the DFA.
7. If mother is abroad, affivadit of support and consent
and photocopy of her passport (front and last page)
must be autheticated by the nearest Philippine Embassy
or General Consulate.
8. If both parents are abroad, person applying in
behalf of the minor must submit a Special Power of
Attorney and affidavit of support and consent authenticated
by the nearest Philippine Embassy or Generate Consulate
plus the original passport of person traveling with
the minor plus DSWD clearance.
NOTE:
Only the mother; father or person applying in behalf
of the minor is required to appear to the DFA if minor
is seven (7) years old and below.
Eighteen
(18) years old up to Sixty Four (64) year old
1.
Birth Certificate - NSO
2. Marriage Contract - NSO (If married woman)
3. NBI (for travel abroad)
4. Original Identification Card (at least 2 ID)

NOTE:
If Birth Certificate has no record, submit the Negative
Record of Birth (NSO and LCR); Joint affivadit
of birth; two old docs showing Date and Place of birth
(for Senior Citizens only).
Supporting

Submit 2 old documents showing date and Place of birth
if the Birth Certificate is late registered.
